Fairfield County Ohio is Hiring a Public Health Nurse (Full-Time or Part-Time) Near Lancaster, OH

Under general direction from Nursing Director, higher-level public health nurse, health commissioner or other administrative superior, visits homes of clients to provide direct nursing care and health education, provides direct nursing care, assists in clinics, presents health educational programs, counsels clients related to health issues, and performs miscellaneous nursing activities.

  • Conduct home visits and clinic services by identifying the physical, social, and emotional health needs of individuals and families through systematic assessment utilizing professional skills, health history, physical evaluation, nutrition and other tools; develop a comprehensive care plan based on the systematic assessment involving the patient, family, co-workers and other disciplines on the health care team. Implement care plan through nursing interventions (conduct interviews and examinations, collect specimens, assess health status, change dressings, administer preventive immunizations, supervise care of home health aide and L.P.N. in home, provide instruction and health counseling). Assist families and clients to assume responsibility for providing and/or arranging for care and self-care. Assist family/client to problem solve. Resolve complaints from clients and family members. Provide health education to individuals and groups. Perform communicable disease follow-up and control.
  • Maintain records on all observations and interventions provided during home visits and clinic sessions. Document accurate and concise information in client’s records and in reports to others. Complete reports and records in timely manner.
  • Ensures that clients receive services in accordance with the Civil Rights Act, the American with Disabilities Act, and other relevant federal, state and local laws.
  • Demonstrates skill in the use of computer software such as word processing and database applications. Ability to collect and organize data sufficient to maintain accurate and complete records, and to prepare reports in accordance with state and federal guidelines. Submit reports in a timely manner
  • Initiates and maintains a medical record containing documentation of consent, emergency contact, screening performed, test referrals and their results, and follow-up of medical problems. Ensures that documentation is organized so that the status of the client’s medical care and any follow-up efforts by staff can be determined at any point in time.
  • Maintains privacy and confidentiality in all services provided to clients.
  • Ensures that patients health education that is documented, culturally sensitive, and presented at a literacy level appropriate for the client.
  • Provides and participates in professional education activities.
  • Completes necessary reporting requirements for all program areas.
  • All other duties as assigned
  • Licensed as a Registered Nurse in the State of Ohio
  • Degree in Nursing from an accredited college or university required. Associate's degree accepted, Bachelor’s degree preferred
  • A minimum of two years of nursing experience or six months or more experience in Public Health preferred
  • Valid Ohio driver’s license.
